COVID-19 Health and Safety Requirements

Arizona Roller Derby (AZRD) prioritizes the health and safety of our members, fans, and families. To ensure our health and safety and the health and safety of others, we have the following COVID-19 Health and Safety Plan* which goes into effect Jan 1, 2022. AZRD requires:

(1) valid, current proof of vaccination against COVID-19 (or proof of negative COVID-19 PCR test) for participation in all events, and 
(2) completion of COVID-19 health check upon entry to events.

COVID-19 Liability Waiver

The novel coronavirus (“COVID-19”) has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. 

Arizona Roller Derby has put in place preventative measures to reduce the spread of COVID-19; however, cannot guarantee that you will not become infected with COVID-19. Further, attending any program or activity operated by Arizona Roller Derby could increase your risk of contracting COVID-19. 

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 while attending Arizona Roller Derby events and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Arizona Roller Derby events may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Arizona Roller Derby employees, volunteers, and program participants and their families.


 

Medical Release Waiver

Medical: It is the responsibility of the undersigned to ensure that the above-named person is medically fit to participate in strenuous on-rink or off-rink activities. As stated below, participation in roller derby activities presents an inherent risk of injury to person or property. The undersigned certifies that the above-named participant has no known conditions that prohibit or limit participation in any derby or skating activities held by or in association with Arizona Roller Derby. Additionally, the undersigned is strongly encouraged to have primary medical insurance in place for the participant to cover any expenses related to any potential injury that may arise from their participation in Arizona Roller Derby. The undersigned also understands that AZRD does not provide personal or accidental insurance coverage to participants – participants SKATE AT THEIR OWN RISK. AZRD members are required to purchase the WFTDA supplemental insurance within 30 days from enrollment.

Equipment and Skates: Participants must wear the following mandatory safety equipment during all Arizona Roller Derby on-rink activities and practices: knee, elbow and wrist pads, helmet and mouthguard. Eyeglasses must have plastic shatterproof lenses. The undersigned must take full responsibility that the above-named participant is wearing the aforementioned safety equipment at all times and that it is properly worn. Only quad roller skates are permitted. All skates must be rink-safe, meaning that their use must not gash, indent or blemish the skating surface or any other surface and that the skates will not cause injury to property or person(s). All liabilities thereof are undertaken by the undersigned.

Conduct: Spectators (patrons on the premises as a result of your involvement in the roller derby) as well as participants must behave in a respectful manner to both person and property. Behavior which could potentially lead to intentional or unintentional bodily injury or injury to property will not be tolerated.
Indemnification and Risk Acknowledgment: In consideration of being allowed to participate in any way in the ARIZONA ROLLER DERBY athletic sports programs, related events and activities, the undersigned acknowledges, appreciates and agrees that:

1. The risk of injuries from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment and personal discipline may reduce this risk, the risk of serious injury remains; and,
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and I assume full responsibility for my participation; and,
3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Arizona Roller Derby, the rinks, coaches, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors and advertisers (“RELEASEES”) WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss, or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.

Health and Safety Statement

Roller derby is a high-speed, heavy contact aerobic sport. Due to the inherent dangers and risks involved (up to and including paralysis and death), AZRD, recommends that each participating member consult with a physician regarding their fitness for participation BEFORE engaging in the sport.  

AZRD does not require a physical or physician’s clearance from its participants upon enrollment, but does reserve the right to request a clearance at any time before allowing a participant to skate.  

AZRD does not require each skating participant carry personal health insurance however it is strongly encouraged. All participants are required to advise AZRD if they have a change in the status of their personal health insurance.

AZRD has a strict drug and alcohol policy – no participants may skate while under the influence of drugs, prescription narcotics or alcohol for their own safety and that of those around them. AZRD reserves the right to prevent any participant from skating under this policy and request drug/alcohol screening or a physician’s clearance at any time.

"People should consult their doctors before beginning competitive sports or an exercise program. Doctors ask about known medical disorders in the person and family members and symptoms the person has. They do a physical examination, including listening to the heart with a stethoscope. This evaluation identifies some of the rare young people who could have a previously unsuspected heart disorder that can lead to serious heart rhythm abnormalities or sudden, unexpected death with strenuous exercise. It also detects conditions that could restrict activities. For example, some individuals are more likely to develop musculoskeletal injuries after activities involving sudden starts and stops (such as tennis and basketball) as well as those that involve impact (such as jogging).

Screening for Sports Participation: Most schools and organized sports organizations require that people have a doctor evaluate whether they can safely participate before they join the program. Doctors ask questions about general health and do an examination as previously described. Adolescents and young adults are often asked about use of illicit and performance-enhancing drugs.(See also the US Anti-Doping Agency web site.)

People older than age 40 who are starting an exercise program should report any diagnoses of heart disorders or arthritis and describe any symptoms of chest pain, shortness of breath, leg pains with walking, palpitations (awareness of heartbeat) or irregular heartbeats, joint pain or swelling, and inability to exercise for long periods (for example, because of weakness, shortness of breath, sweating, or leg pains). Certain drugs may limit the ability to exercise, such as beta-blockers, which slow heart rate, and sedatives, which can cause drowsiness and increase the risk of falling.

Conditions that make exercise too risky to recommend in children include heart inflammation (myocarditis), which is uncommon. It increases the risk of sudden death due to heart dysfunction. Fever is another, because it impairs ability to exercise, may be a sign of serious illness, and may lead to heat-related illness such as heatstroke. Conditions that lead to dehydration (for example, vomiting and diarrhea) are also risky, because sweating during exercise can worsen dehydration. Conditions that make exercise too risky to recommend and that occur mainly in adults include angina pectoris and a heart attack in the previous 6 weeks. People should take precautions when they have certain other conditions. For example:

- People with cystic fibrosis or diabetes, which can lead to dehydration, should drink large amounts of fluids around the time of exercise.
- People who have had several concussions or a recent concussion should avoid contact sports. People with seizures should avoid swimming and weight lifting alone to prevent injuring themselves and should avoid riflery and archery to prevent injuring others.
- People with an enlarged spleen (for example, after infectious mononucleosis) should avoid contact sports because being hit or falling may rupture the enlarged spleen."

Confidentiality Agreement and Non-Compete Clause

This agreement is between Arizona Roller Derby, an Arizona 501(c)(3) non-profit organization (aka AZRD), (Employer/League), and Participant of AZRD. In consideration of Employer's/League invitation to be a participant for and on behalf of, Participant agrees as follows: 

1. Agreement Not to Disclose Confidential Information
I, the Participant, acknowledge that Employer/League may disclose to me or give me access to confidential information so that I may perform my volunteer duties. I agree that the confidential information includes Employer/League's trade secrets, sales and profit figures, customer lists, sponsor lists, member lists and contact information, marketing strategy, endorsement strategy, relationships with contractors, customers or suppliers, and opportunities for new or developing business. The confidential information may be contained in written materials such as computer hardware and software, disks, documents, files, drawings and product specifications. It may also consist of unwritten knowledge, including ideas, research, processes, practices or know-how. While I am a participant for the Employer/League, and afterward, I will not use or disclose to any other person or entity any confidential information or materials (either written or unwritten) except when I am required to do so to properly perform my duties to Employer/League or as required by law. Information in the public domain, information generally known in the trade and information that I acquire completely independently of my services for Employer/League is not considered to be confidential.

NOTE: As an AZRD volunteer all emails and correspondence are to be considered confidential information. Blind copying, or deliberate forwarding of emails to outside parties is unacceptable by league standards.

2. Return of Confidential Information
Upon the termination of my voluntary work, or at any time that Employer/League requests it, I will immediately return all confidential information and materials to Employer/League.

3. Right to an Injunction
I acknowledge that in addition to receiving or having access to confidential information as part of my participant efforts, I will be in a position of confidence and trust with other participants, clients and customers of Employer/League, and fans. I acknowledge and agree that if I breach or threaten to breach any of the terms of this Confidentiality Agreement, Employer/League will sustain irreparable harm and that Employer/League will be entitled to obtain an injunction to stop any breach or threatened breach of this agreement.

4. Reasonableness
I acknowledge that the restrictions in this agreement are reasonable and necessary to protect Employer/League and its confidential information.

5. Survivability
This agreement will survive the termination, for any reason, of my participant efforts with Employer/League.

6. Entire Agreement
This is the entire agreement between the parties. It replaces any and all oral agreements between the parties, as well as any prior writings.

7. Governing Law
This agreement will be governed by and construed in accordance with the laws of the state of Arizona.

8. Waiver
If one party waives any term or provision of this agreement at any time, that waiver will be effective only for the specific instance and specific purpose for which the waiver was given. If either party fails to exercise or delays exercising any of its rights or remedies under this agreement, that party retains the right to enforce that term or provision at a later time.

9. Severability
If any court determines that any provision of this agreement is invalid or unenforceable, any invalidity or unenforceability will affect only that provision and will not make any other provision of this agreement invalid or unenforceable and shall be modified, amended or limited only to the extent necessary to render it valid and enforceable.

Code of Conduct

1. General
All members of AZRD, including non-competing members, are expected to contribute to a positive and safe atmosphere. Members may be placed on Probationary or Inactive status by the Executive Officers or Compliance Chair if they display behavior contrary to this expectation.  These behaviors include but are not limited to:

- Disregard for the physical safety of League member(s)
- Harassment or verbal threats towards League member(s)
- Sexual advances towards League member(s)
- Disregard for League policies and procedures
- Insubordination or disrespectful behavior towards other league member(s)
- Unprofessional or inappropriate comments regarding skater(s), refs, fans, etc., on any form of social media or external websites.

2. Professionalism
All members, including non-skating members, are representative of the League in its public dealings, and as such are expected to interact with Sponsors, Vendors, Venues, and Attendees of official AZRD functions as professional and appropriate to the circumstances of that business relationship.

3. WFTDA Bouting Rules and Regulations
When playing WFTDA, AZRD adopts and plays by the WFTDA standardized rules as they evolve and incorporate into official rules. AZRD adopts and follows all operational rules for league membership as they evolve. All members, including non-competing members, are required to adhere to these guidelines as a condition of membership.

4. Confidentiality
Per AZRD’s agreement with the WFTDA and League Confidentiality Agreement, all AZRD and WFTDA documents, training methods, emails and correspondence are to be considered confidential information. Blind copying, or deliberate sharing of documents, methods, and emails to outside parties is unacceptable by League Code of Conduct and may result in disciplinary action up to and including expulsion from the League.

5. Violations of Code of Conduct
All members, including non-competing members, who violate the Code of Conduct are subject to a change in status, as prescribed in this document, upon written notice by the Executive Officers or Compliance Chair.
